The staging of damage is the result of the attacker's success in attacking, and the defender's success in defending, both put in relation to eachother, resulting in damaged being raised or lowered in favour of the one with the most success. So if the attacker is attacking better than the defender is defending, then the damage will be raised, and vice versa.

To many people's dismay, staging of damage happens at different points in the Combat Sequence depending on it being ranged or melee combat. Different things have to be taken into account, and some optional rules are simply nullified or made impossible.
